Scholarship Renewal Application

Who can apply:
- Master's students entering Year 2
- Ph.D. students entering Year 2 / 3 / 4
- Students who missed the previous renewal may submit a make-up application for one semester

| 1. Application Form (with advisor signature) | Download the template, fill in your information, then sign by hand. Download link at the bottom of this page. |
| 2. Terms of Agreement | Must be signed. Download link at the bottom of this page. |
| 3. Official English Transcript (all semesters) | Please print after July 13. Must show numeric scores, total credits, and GPA. The red machine is on the 2nd floor of the administration building and at the Joint Service Center. |
| 4. Safety Seminar Certificate | Certificate must be issued after 2025 Fall semester. If you don't have one, contact Mr. Jesse. |
| 5. Copy of ARC | ARC must show you are a student at NTUT. |

| 10. Copy of Bank Account / Passbook Cover | Please upload the bank account cover page or passbook copy. |
